CFI is available to eligible seniors and adults living with chronic illnesses or disabilities who meet both medical and financial requirements established by New Hampshire Medicaid.

Depending on your approved care plan, services include personal care, meal preparation, homemaking, errands, medication reminders, mobility assistance, companionship and respite care.
